Education and Experience:
- Bachelor’s Degree in Behavioral Health, Psychology, Health Behavior Science, or related field
- Experience working with adults with severe and persistent mental illness and/or substance use disorders
- CPR and First Aid Certification
- Valid driver’s license and permission for a review of driving record to meet insurance requirements
Responsibilities:
- Professionally participate in team conferencing to create and implement care plans for clients
- Maintain a therapeutic alliance with clients
- Refer and link clients to all needed services provided by an outside agency
- Coordinating independent living assistant, medical and vocational services
- Work with community agencies to develop needed resources including housing, employment options, and income assistance
- Provide support and consultation with the client’s family
- Provide in-home services including ADL training, medication assistance and training, social support, and home management assistance; providing supportive counseling
- Document and keep other staff, clinical coordinators, and Program Director informed of all changes in the consumer’s mental or social status
- Carries primary or secondary cell phone as assigned and responds to after-hour and emergency calls in accordance with program directives
- Provides education and work as an advocate for persons served in the communit.
