We are currently seeking qualified candidates to fill the role of Direct Care Professional in the Exeter, Kutztown, and Allentown areas. We offer 8 hour shifts: Approx. 8a-4p, 4p-12a and 12a-8a.
Responsibilities of the Job include:
- Responsible for the safety and well-being of all the individuals residing in the home(s).
- Perform all daily activities within home setting including, but not limited to, meal preparation, activities of daily living, shopping, household cleaning and maintenance, leisure activities, and consumer outcomes. Administer medications to consumers.
- Drive consumers to medical appointments, community activities, school/day programming, religious services, and family visits.
- Accurate completion of required paperwork including medication logs, daily logs, goal logs, incident reports, sleep / awake charts, weight charts, fire drill logs, seizure logs, behavior plan documentation, gas receipts, vehicle logs, consumer funds.
