In this role, you will commute to various homes throughout the Mifflin County area and assist adults with behavioral health and intellectual development disabilities in their homes.

Job Duties:
- Perform the delivery of services with this organization and our consumers to include:
- Consumer transportation to include but not limited to medical appointments, financial and benefit appointments, housing appointments, community and outcome directed activities, school / day programming, and religious services.
- Perform all daily activities within any home setting and / or community setting.
- Assist/mentor consumers with activities of daily living / perform activities of daily living.
- Perform medication administration.
- Assist with ISP development and outcomes.
- Accurate completion of required paperwork to include but not limited to timecards, progress notes, schedules, assessment forms, medical appointment forms, daily logs, medication logs, vehicle logs and expense forms.
- Assist with on call activities and emergency situations as requested by the Program Coordinator.
- Responsible for the safety and well-being of all the individuals residing in the home.
- Work with the Program Coordinator to ensure that all needs of the individuals are bet according to their ISP.
- Assist the Program Coordinator with special projects as related to the agency.
Requirements:
- Must be 18 years or older, have a valid driver’s license, and have a car with valid insurance and registration.
- All candidates must be reliable, free from child abuse offenses and able to pass a drug test.
- Knowledge of and ability to work with developmentally disabled persons.
- Excellent communication skills.
- Excellent writing, documentation, financial, and math skills.
- Excellent organization skills.
- Team Player.
